Occupational Medicine is located in Suite 310 of the Centre Medical Science Building. The building is located at 1850 East Park Avenue, State College, PA 16803.

Ample parking is available at no charge to the clinic location.

How do I get there if I don't drive?

Bus service via CATA's "P Route" is available. CATA schedules are online at www.catabus.com or by phone at 238-2282.

Do I need to complete any forms?

Yes. Your supervisor and you will need to complete the form EMPLOYERS REPORT OF OCCUPATIONAL INJURY OR DISEASE. This form is available through the Workers' Compensation web site.

It must be completed for submission to Workers' Compensation.

What other services does Occupational Medicine provide?

Occupational Medicine is comprehensive occupational and environmental medicine practice specializing in prevention based non-emergent and injury care. Our practice also includes toxicology and exposure assessments, health surveillance exams and non-surgical orthopedic care.
